Our regular office hours are 8:30 a.m. to 5:00 p.m., Monday through Friday. Our staff is available during office hours for appointment scheduling and general questions.

New patient forms are available on our website for your convenience.

Prior to your first appointment, we will attempt to reach you by phone for registration. If registration by phone is inconvenient or we have not been able to contact you by phone in advance, please arrive 15 minutes before your scheduled appointment so that this process can be finalized before your appointment time.

Reminder: Bring your insurance card and identification with you to your first appointment and whenever your coverage change.

Patients should bring their up-to-date medication list or all of their prescription bottles. Most of the time we don’t need over-the-counter medications, inhalers, or eye drops on the list, unless the physician asks for them specifically.

For insurance, billing, and payment questions, our representatives are available to assist you by phone or in person. To speak to a representative, contact our Business Office at 713-464-9100 between 7:00 a.m. and 5:00 p.m.

If your insurance plan requires a referral or authorization, please make sure we have received this referral prior to your appointment or we may have to cancel your appointment. If you have questions regarding your insurance plan or policy limits, please contact your insurance company.

Patients should try to get their lab work done at the time recommended by their physician or within 1 to 2 weeks prior to their appointment. If  unable to do the lab prior to the appointment, please contact the Clinic for further instructions.

When the Clinic is closed, patients may reach the on call physician, or the physician designated to take his/her calls by dialing their main Clinic telephone number, (Katy and Memorial Area 281-579-5799) (Cypress and Willowbrook Area 281-890-9944). Our answering service will contact the on call physician.

Patients should contact their pharmacy with prescription refill requests, instructing their pharmacy to electronically submit refill requests to the Clinic. The pharmacy or pharmacist will contact the Clinic on your behalf to obtain authorization from your physician for prescription refills.

Please try to contact the clinic during working hours for a prescription request to be completed in a timely manner and do not wait till the last day to request the refills. As a group policy we do not refill any controlled substance electronically or over the phone.

Renal Clinic patient portal is a secure online website that gives patients convenient 24-hour access to personal health information from anywhere with an Internet connection. Using a secure username and password, patients may view health information such as:

  • Recent doctor visits
  • Medications
  • Educational materials
  • Laboratory results

You may also do the following:

  • Exchange secure e-mail with our clinic (NEVER TO USE IN AN EMERGENCY OR URGENT MATTER)
  • Request prescription refills
  • Update contact information
  • Download and complete forms, when available

If you haven’t received your log on information, please contact our office. We will provide you with instructions for setting it up. There may be a couple of steps involved in setting up your account, including creating a secure password. This is to make sure only you have access to your health information.

Patient portal has privacy and security safeguards in place to protect your health information.

  • To make sure that your private health information is safe from unauthorized access, patient portal is hosted on a secure connection and accessed via an encrypted, password-protected logon.

NOTE: Although patient portals use safeguards, there are other safety tips you should follow when accessing the patient portal. Always remember to protect your username and password from others and make sure to only log on to the patient portal from a personal or secure computer.
